DISPATCH COMPLAINT — Traumatic Injury
PAI– Broken Wrist will be sitting on the bench in the Hockey Rink
AOS — To Find a 14 Y/O/M sitting on a bench with an obvious deformity to the Right Lower Extremity.
HPI/MOI — Patient stated that he was playing hockey when he was checked open ice. His wrist was caught between his chest and stick when the other player made contact with him.
DAILY MEDICATIONS — NONE

NO DEFICITS NOTED, SPEECH CLEAR, ANSWERS APPROPRIATELY.
SKIN–INTACT, WARM, DRY, NORMAL APPEARANCE, GROSSLY ATRAUMATIC, ACYANOTIC
AIRWAY– NATURALLY PATENT, ABLE TO SPEAK IN FULL SENTENCES
HEENT–PERRL, (-) DCAP BLS TIC (-) JVD. TRACHEA MID-LINE, NOSE EARS AND MOUTH FREE OF FLUID/BLOOD.
CHEST– (-) DCAPP BLS TIC STABLE, SYMMETRICAL EXPANSION, LUNGS CLEAR PRESENT ALL FIELDS.
ABD– (-) DCAPP BLS TIC SOFT NON-TENDER
PELVIS–(-) DCAP BLS TIC, STABLE
GU/GI– NO INCONTINENCE NOTED. NO COMPLAINTS.
EXTREMITIES– (+) Deformity in the Lower Right Extremity (+) DISTAL PMS.
INITIAL VITAL SIGNS– BP: 120/70 P: 80 R: 18
TREATMENT– Patients Hockey Pads were removed and his shirt was cut to expose the affected area. Patients Arm was immobilized via a SAM Splint, Roller Gauze, & Tape and held in place sling formed using a triangular bandage and rested on a pillow.
TRANSPORTED– Assisted Patient onto the litter where he was secured and moved into MICU 9-1. Patient was then transported to University Hospital – Hershey (per Fathers Request). Upon arrival report was given to the Charge Nurse and the patient was moved onto a Hospital Bed where care was transferred to Hospital Staff.
OUTCOME– PT REMAINED STABLE/UNCHANGED THROUGHOUT ENCOUNTER
